Our free in-home assessment is how we are able to gather all of the information and learn about you and your needs. Once an initial phone call is completed, a Care Manager will schedule a time to meet with you to complete the assessment. The in-home assessment will take approximately 60 – 90 minutes as we explore your daily routines, medical issues, favorite activities, care needs, and more.
